Open the PDF and read it. If something is wrong, fix it here and download again. Sign by hand. You sign under penalty of perjury, so everything must be true.
- Attach it to your SC-100
Put SC-100A behind your SC-100. On your SC-100, check the box in item 1 (more than two plaintiffs) or item 2 (more than one defendant). Attach any other forms you said you would attach (for example SC-103).
- Make copies and file
Make one copy of all pages, including this attachment, for each defendant, plus one copy for you. File them with the small claims clerk with your SC-100. How to file.
- Serve every defendant
Each defendant must be served with a court-stamped copy of the claim and its attachments. How to serve.
